MUCAI Dance Academy
Using the Arts to Educate

I understand and accept that all MUCAI Dance Group participants must take at least 2 full dance classes all year, with at least an 80% attendance rate. Failure to do so will result in removal from the program.*

I understand and accept that performance opportunity is at the descretion of the teachers and the rules governing each performance event. *

I understand and accept that because participation is free, an adult from my family must volunteer for at least one promotional event, each year. *

I understand and accept that I or my child can be dismissed from MUCAI Dance Group if not folowing school policies regarding attendance, behavior, or for not showing up at events we have committed to perform in or help with.*

I understand and accept that MUCAI Academy does not chaufer performers to events within the cities normally served (Columbus, New Albany, Worthington, Westerville, Dublin, Delaware, Canal Winchester, Obetz) and that I am responsible for getting myself or my child to and from these events. I must complete a separate field trip permission form for out of town events. *

I allow MUCAI Academy to use photos and/or videos of my child's performances for promotional use, including posting to social media platforms. *

I allow MUCAI Academy and it's representatives permission to administer First Aid/CPR or send to the nearest medical facility, in the event of an injury or health crisis. I further hold MUCAI Academy and its representative harmless for any injury or loss due to participation in events or attempts to administer First Aid/CPR. *
